Vermont Electronic Death Registration System (EDRS)
The Vermont Department of Health is developing an Electronic Death Registration System. This web-based application will allow physicians, funeral directors, and others involved in reporting deaths to enter information directly into the system, eliminating the need to coordinate signing of the certificate and delivery to a town clerk. This more timely and efficient process will benefit those awaiting the final document and its data, including survivors and public health planners. When fully implemented, the EDRS will also result in more accurate and consistent information about deaths in Vermont.
For deaths occurring on July 1, 2008 and later:
- Deaths will be reported using a form known as the Preliminary Report of Death (PROD).
- The PROD can be used in place of the Death Certificate for obtaining necessary Burial Transit and Cremation Permits.

Funeral directors will need to fax the completed and signed Preliminary Report of Death directly to Vital Records for data entry and registration in the EDRS. FAX # 802-651-1787.

Once all permits are obtained, the signed Preliminary Report should be forwarded to town clerk of the town where the death occurred.

Certified copies of Death Certificates can be obtained from town clerks in either the Town of Residence or Town of Death, but please call ahead to assure they have received the certificate. We will strive to have copies available to clerks as soon as possible, but are aware there may be some delays during the transition.

The VDH plans to introduce the EDRS to physicians and funeral directors in phases during 2008 and 2009.
As the application begins to take shape, the EDRS staff will invite representatives of the various groups who participate in death reporting to test the application and related business practices. This includes funeral directors, physicians, town clerks, and public health programs dependent upon the information reported in death records.
